Aledade, the largest US network of independent primary care, has added more than 500 new primary care practices, clinics and community health centers to its network for 2025.
Aledade now supports more than 2,400 primary care organizations nationwide, serving nearly 3 million patients. It works with value-based care programs, including the Medicare Shared Savings Program (MSSP), Medicare Advantage, Medicaid and commercial insurance.
Eighty-three percent of Aledade’s new MSSP participants are joining the program for the first time, which shows Aledade is helping expand value-based care in the Centers for Medicare & Medicaid Services’ (CMS) flagship program. Nearly one in five new MSSP participants receive support from Aledade.
Aledade also helps CMS test and improve alternative payment models. It increased participation in ACO REACH, a pilot program for risk-sharing and capitated provider payments, by 13%. Aledade is the top participant in CMS’ new Primary Care Flex Model, a voluntary program testing advanced payments for MSSP-participating practices.
Where Aledade works
Aledade operates in cities and rural communities across 46 states and Washington, DC. It works with primary care organizations of all sizes, including over 300 community health centers. Nearly two-thirds of these centers are in areas where the US Health Resources and Services Administration has identified a shortage of healthcare providers.

Saving healthcare dollars
In 2023, Aledade’s accountable care organizations (ACOs) saved a record $801 million through the MSSP. This was the highest amount among all accountable care partners nationwide. Aledade-led ACOs also achieved higher quality scores than non-Aledade MSSP participants.
Ninety-three percent of Aledade’s physician-led ACOs saved money, compared to under 70% of non-Aledade participants. Since 2014, Aledade and its partners have saved US taxpayers more than $2 billion.
