Rules for signature stamps

I would like the doctors in my office to use a rubber signature stamp for progress notes and reports. What is the correct way to implement the use of these types of stamps?

Many states have their own rules concerning signature requirements and you should check with your state board.

Medicare says that if the note is clear that the doctor performed the service, a signature stamp is acceptable. It is presumed that the doctor performed the service if the note is in the doctor's handwriting.

If the note is typed, it should indicate that it was “dictated by...” and list the doctor's name and initials. If the note was written by another person, it must state that it was dictated by the particular doctor.

A computer-generated report must indicate that the doctor has read the report. This is accomplished by the doctor making comments on the report, if any, or else by writing the word “agree” on the report or by initialing or signing the report.

The stamp must be personally applied by the doctor, must remain in the doctor's personal possession and may not be shared with any other provider.
